Terms and Conditions – VIP

Terms and Conditions for Monthly Membership *DermaDoc SKIN CLUB

This agreement (“Agreement”) is entered into by and between DermaDoc/Antiwrinkle Galway Ltd. and the individual subscribing to the monthly membership (“Member”). By enrolling in the monthly membership, the Member agrees to the following terms and conditions:

1. Membership Activation and Payment Terms*
   1. Membership activation is subject to receipt of payment or an active Direct Debit instruction, which may take between one and four working days.
   2. Membership subscriptions are non-refundable and non-transferable.
   3. Subscriptions are subject to a minimum term of 12 months, and any unused portion of the subscription period cannot be refunded or carried over to the following month.
2. Appointment Attendance and Subscription Conditions*
   1. It is the Member’s responsibility to schedule and attend the treatments within the membership month.
   2. Members are required to contact DermaDoc/Antiwrinkle Galway Ltd at least 72 hours (3 days) prior to the appointment for rescheduling or cancellation. Failure to do so may result in the loss of the scheduled treatment for that month.
   3. Missed appointments will result in the forfeiture of that treatment for the month.
   4. Appointments must be scheduled no more than 1 month in advance.
3. Direct Debit and Membership Suspension*
   1. Cancelling the Direct Debit instruction with the bank is the responsibility of the Member.
   2. If any monthly direct debit payment is not received on the due date, the membership will (except in exceptional circumstances and at our sole discretion) be automatically suspended until all due payments are brought up to date.
4. VIP Appointment Usage and Membership Misuse*
   1. VIP appointment usage, including the “monthly free treatment,” is dependent on maintaining an active monthly subscription.
   2. Using the membership by anyone other than the enrolled Member may result in the suspension or cancellation of the membership.
5. Changes to Terms and Conditions*
   1. DermaDoc/Antiwrinkle Galway Ltd reserves the right to modify these terms and conditions at any time. Members will be notified of any changes in advance.
   2. By enrolling in the monthly membership, the Member acknowledges that they have read, understood, and agreed to comply with these terms and conditions.
